How and when should we use Patient View?

The two most common ways to access and use Patient View:

1) From the Aligned Patient Map on your Overview screen, you can drill down to Patient View to get more detail on individuals in your panel.

  • To access Patient View from the Aligned Patient Map on your Overview screen, you can select the hexagon that corresponds to the Patient you would like more information about, then click “View Profile.”

  • You can also navigate to Patient View from the Overview screen by selecting a patient in the patient list, then clicking “View Profile.”

2) Physicians can also access Patient View by using the search bar at the top of the screen. Searching in this bar will take you to the aligned patient list, where you can select the patient and click “View Profile.” You can take this approach to prepare for your upcoming Medicare appointments alongside your EMR to scan for any surprises:

  • Medications: Anything new that should be reconciled?

  • Diagnoses: What may need to be re-evaluated?

  • Recent Activity: Is there anything that you may want to ask your patient about?
