What is the "Do Now Alerts" Metric?

The Do Now Alerts metric is a measure of your team's effectiveness in addressing urgent clinical needs. It shows the percentage of high-priority "Do Now" alerts that were successfully acted upon within the calendar month. A higher percentage indicates more proactive and timely patient care, which is crucial for improving outcomes and performance in value-based care.

How the "Do Now" Metric is Calculated

The Do Now Pillar Metric is calculated by dividing the Number of Successful Suggested Actions by the Total Number of Do Now Alerts and multiplying the result by 100.

Let's break down how the two parts of this calculation (the Denominator and the Numerator) are determined.

Denominator: Total Do Now Alerts for the Month

This number represents all the "Do Now" alerts that your team was responsible for addressing during the specific calendar month.

We count a "Do Now" alert in the denominator if it meets these criteria:

  • Alert Timing: The alert was either:

    • Carried over from the previous month, OR

    • Newly generated within the current month.

  • Month-End Status: The alert was still considered open or due by the last day of the month. This includes alerts that were overdue or had a due date falling on the last day of the month.

  • Alert Uniqueness:

    • For ADT alerts (Admission, Discharge, Transfer): Each separate ADT event for a patient that triggers a "Do Now" alert is counted individually. A patient could have multiple ADT-related "Do Now" alerts in a month, and each would contribute to the denominator.

    • For Non-ADT alerts: We only count one instance of the same alert type per patient within the month, even if it expired and reappeared.

  • Patient Alignment: The patient associated with the alert was aligned to a provider for at least one day during the month.

    • Important Note: Alerts for patients marked as "Not My Patient" or who were fully de-aligned for the entire month are excluded. However, if a patient was aligned for any part of the month, their Do Now alerts are included, even if they were de-aligned later.

Numerator: Successful Suggested Actions

This number represents the "Do Now" alerts from the denominator for which an eligible action was logged within the calendar month.

An action is counted in the numerator if it meets these criteria:

  • Eligible Action Types: The action taken on the "Do Now" alert was one of the following:

    • Scheduling a new appointment for the patient.

    • Identifying a previously scheduled appointment that already addresses the alert's clinical need.

    • Marking the patient as "Not My Patient (NMP)" specifically with the reason being "In SNF" (Skilled Nursing Facility) or "In Hospice." These specific NMP reasons indicate the patient's care is being managed elsewhere appropriately for the alert type.

  • Timing of Action: The eligible action was logged in the platform within the calendar month, while the "Do Now" alert was active.

  • Special Case (Alerts without Due Dates): If a "Do Now" alert does not have a specific due date, any eligible action taken while that alert was active during the month will be counted in the numerator.

  • Exclusions from Numerator: Actions are not counted in the numerator if:

    • The patient was marked as "Not My Patient" with a generic reason (not specifically "In SNF" or "In Hospice").

    • The patient was fully de-aligned from your panel when the action was taken.


Understanding the Data Download

The CSV data download provides a detailed list of "Do Now" alerts and associated actions, designed to help you understand the data that feeds the "Do Now" Pillar Metric and replicate the calculation if needed.

Here's what you'll find in the CSV file and how the data is organized:

Inclusion Rules for the Data

The data download includes "Do Now" alerts and relevant actions that occurred within the past 90 days from the date you generate the download.

  • All "Do Now" alerts that were raised within this 90-day window are included.

  • Any actions logged on those "Do Now" alerts within the 90-day window are included.

Handling of ADT Alerts in the CSV (Deduplication)

ADT (Admission, Discharge, Transfer) events can sometimes generate multiple alerts for a single patient event. To make the CSV easier to use and align with how we think about managing a single hospital stay or transfer, we apply a deduplication logic for ADT alerts:

Prioritization for Display: Only one row will be shown in the CSV for each unique ADT event, based on this priority:

  • If an eligible action was taken: The row displayed will be the alert entry that is directly tied to a successfully completed eligible action (e.g., an appointment scheduled after a hospital discharge). If multiple eligible actions exist for the same ADT event, the row tied to the latest completed action is shown.

  • If no eligible action was taken: The row displayed will be the latest "Do Now" alert generated for that specific ADT event.

This approach ensures that the CSV reflects the most relevant alert and action (if one occurred) for each distinct hospital or transfer event.

Other Notes about the CSV

  • Non-ADT Alerts: For all other types of "Do Now" alerts (not related to ADT events), each qualifying alert and any associated eligible actions within the 90-day window will be listed as individual rows. There is no grouping like with ADT alerts.

  • Due Date Status: Alerts are included in the CSV regardless of whether they were overdue, due on a specific date, or had no due date, as long as they fall within the 90-day inclusion window. The CSV provides the data, and you can filter or analyze based on due date status as needed.

While the download provides detailed data, remember that the "Do Now" Pillar Metric calculation has specific criteria related to the calendar month and end-of-month status that are applied to this data. The CSV gives you the raw material over a 90-day window to help understand the components of the pillar metric and your team's performance.
