Do Now Alerts
This pillar is a measure of your team's response to patients with "Do Now" alerts. Scheduling appointments for these patients in a timely manner can help avoid costly readmissions, improve outcomes, and drive value for your practice.
How is this calculated?
A rolling 4-week average of completed actions logged for patients with Do Now (red) alerts.
What alerts count toward the Do Now pillar?
Admission, Discharge, and Transfer (ADT) alerts
Predictive alerts for Unplanned Admissions for Multiple Chronic Conditions (UAMCC) and Preventable ED Visits
Skilled Nursing Facility (SNF) Length of Stay
High Priority Annual Wellness Visits (AWVs)
What actions count toward the Do Now pillar?
Taking the action suggested in the alert and logging it will count. “Scheduled a new appointment” will be the most common (and most impactful) suggested action for driving performance. In these cases “Already Scheduled” will also count.
Proactive Management
This pillar is a measure of the blue portion of your panel with no suggested action.
How is this calculated?
A rolling 4-week average of the percentage of your patients with a light blue or dark blue hexagon.
How can I improve this metric?
Logging actions for your patients with red or pink alerts will change their status to blue.
Why does Proactive Management matter?
Proactive management can help to address chronic conditions, potentially avoiding costly utilization and unplanned admissions.
Proactive patient touchpoints can help to build stronger relationships and increase patient engagement, which can improve the CAHPS Quality Measure in ACO REACH and MSSP
AWV Completion
This pillar is a measure of your completed and scheduled AWV rate over the past 12 months.
How accurate is this?
To determine a patient's AWV eligibility, Pearl will review claims data received for that patient and any platform actions indicating a completed AWV. Due to the delay in receiving claims data from CMS (up to several months), you may see patients marked as eligible for an AWV who have already been seen.
Why do AWVs matter?
AWVs help you provide proactive patient care — they identify potential health issues that could be much more costly to treat down the road.
AWVs are the key to properly capturing existing diagnoses and conditions, which inform CMS of how a practice’s patient panel should be risk weighted.
They help establish an accurate benchmark, which determines a practice’s shared savings opportunity in VBC programs.
