Do Now Alerts
This pillar measures your team's response to Do Now patient alerts on Pearl. Our data have shown that taking the suggested action(s) associated with each alert in a timely manner can help avoid costly readmissions, improve patient outcomes, and drive value for your practice.
How is this calculated?
The Do Now Alerts pillar is calculated on a monthly basis, from the first to the last day of each month.
What alerts count toward the Do Now pillar?
Admission, Discharge, and Transfer (ADT) alerts
Predictive alerts for Unplanned Admissions for Multiple Chronic Conditions (UAMCC) and Preventable ED Visits
Skilled Nursing Facility (SNF) Length of Stay
High Priority Annual Wellness Visits (AWVs)
Which actions count toward the Do Now pillar?
Taking the suggested action(s) for each alert will count towards this pillar. While there are various suggested actions, ensuring an appointment exists with a patient in a recommended timeframe will be our most common suggestion.
Panel Management
This pillar measures your team's ability to keep your Pearl patient panel in the blue β by closing out Do Now and Do Next alerts.
How is this calculated?
The Panel Management pillar is a rolling 4-week average of the percentage of your patients with a light blue or dark blue hexagon. Your Pearl panel is tallied each week, and an average is then taken across the past four weeks.
How can I improve this metric?
Logging actions for your patients with red or pink alerts will change their status to blue.
Why does Proactive Management matter?
Proactive management can help to address chronic conditions, potentially avoiding costly utilization and unplanned admissions.
Proactive patient touchpoints can help to build stronger relationships and increase patient engagement, which can improve the CAHPS Quality Measure in ACO REACH and MSSP
AWV Completion
This pillar measures the percentage of completed and scheduled Annual Wellness Visits (AWVs) across your Pearl patient panel over the past 12 months.
How accurate is this?
Patient AWV eligibility is determined through available claims data and logged actions on the Pearl platform.
Due to delays necessary for CMS to process and adjudicate claims, you may see patients who have already been seen continue to be marked as eligible for an AWV.
Why do AWVs matter?
AWVs help you provide proactive patient care β they identify potential health issues that could be much more costly to treat down the road.
AWVs are crucial for documenting existing diagnoses and conditions of your patients, which help CMS establish an accurate benchmark for your practice next year.
