What are Chronic Care Management (CCM) Alerts
Pearl’s CCM alerts identify patients with multiple chronic conditions who may be eligible for CCM services. The alert is triggered if a patient with multiple chronic conditions has not received CCM in the past 6 months, based on claims data. These alerts are sent at the start of each month. CCM-eligible patients may also be highlighted in Unplanned Admission Alerts.
