What is a Potential Future Admission Risk (UAMCC) Alert?


Pearl's UAMCC alerts use a predictive algorithm to identify patients with multiple chronic conditions who may be at risk for a potentially preventable admission. Patients who are due for an Annual Wellness Visit (AWV) or who may benefit from Chronic Care Management (CCM) are highlighted. Alerts are sent at the beginning of each month

Factors used by the predictive algorithm to identify at-risk patients:

  • Historical utilization patterns

  • Chronic conditions

  • Risk score

  • Frailty cohort

  • Other factors

Recommended Actions

  • Contact patient within 2 days

  • Schedule an appointment or have discussion in 7-14 days to review:

    • Health status and risk

    • Chronic conditions

    • Admission risk

  • Suggest chronic care management if warranted

The Importance of Taking Action

Co-morbid chronic conditions can deteriorate patient health, making patients more susceptible to complications that could lead to an unplanned hospital admission.
