What Potential Preventable ED Visit Alerts mean

Pearl’s Potential Preventable ED Visit alert uses an innovative predictive algorithm to identify patients at risk for a future, preventable ED visit. The algorithm considers factors such as historical utilization patterns, chronic conditions, risk score, frailty cohort, and others to forecast potential ED visits and help PCPs take action to possibly avoid those visits.

Alerts are sent at the beginning of each month.

Recommended actions

  • Contact patient within 2 days

  • Schedule an appointment or have discussion in 7-14 days to review health, chronic conditions, and appropriate times to seek care at the hospital vs. PCP office
