Once I join Pearl, how do I terminate from my other ACO?

Information around ACO Termination

The Centers for Medicare and Medicaid Services (CMS) requires that you

terminate participation with your current or former ACO to participate in

ACO REACH with Pearl Health for the next performance year. Providers can not participate in more than one APM simultaneously, and Medicare will honor historical participation should a provider be present on more than one submission list. Failure to obtain proof of ACO termination could result in rejection from program participation by CMS.

Below are the steps to obtain proof of termination with your former ACO or MSSP:

Step 1: Initiate a communication in writing of your intent to terminate your participation. Initiate a communication via email of your intent to be removed from their participation list immediately and terminate your participation effective December 31st of the year before the performance year starts (ex. If you are participating in Pearl for 2024, the effective date will be 12/31/2023).

Step 2: Obtain evidence from your former ACO rep reflecting your termination from the CMS 4i Portal or ACOMS for MSSP:

Step 3: Forward screen shot of termination from the CMS 4i or ACOMS Portal to help@pearlhealth.com.

In October, we will communicate your approval to participate and continue preparing for your first PY!
