Identifying patients who are aligned under the ACO REACH model

Both claims and voluntary alignment will be visible in the Pearl platform once we ingest the Medicare data. CMS has scheduled delivery towards the end of January, and your patient list should be visible in the Pearl platform in February once we’ve had an opportunity to ingest the data.

Medicare uses a specific formula to determine which of your patients will be aligned to you based on claims history. For the performance year you're participating in, CMS looks at a historical alignment period between July of the two years prior up to June of the year prior (e.g. for the 2024 performance year, CMS is looking at the period between 7/1/21 - 6/30/23) to determine which Medicare patients had the plurality of primary care services provided by a particular practice (based on primary care spend). If Medicare identifies a patient who saw your practice most frequently, then that patient is aligned to the you via claims-based alignment. It is important to note that Voluntary Alignment takes precedence over claims-based alignment.

You can go into the Pearl Platform to see which patients are aligned to you by following these steps:

2. Click on the Patients tab on the top of the screen.

3. You can click on the Alignment tab to sort patients that are claims aligned and voluntary aligned.
