Removing patients from list

Pearl will let you know on a quarterly basis who your net newly aligned patients are. Practices interested in aligning patients must submit voluntary alignment forms 45 days prior to the next quarter. Once submitted, it may take about four weeks after the start of the quarter to receive information on which patients are aligned. In order to see which patients are voluntary aligned in the Pearl Platform, follow these steps:

2. Click on the Patients tab on the top of the screen.

3. You can click on the Alignment tab to sort patients that are claims aligned and voluntary aligned.

Payment for aligned patients will be paid out a month after the quarter begins, but will include payment from the previous month.

Each month, Pearl receives an updated Beneficiary Alignment Report (affectionately known as the “BAR”) from CMS that includes a list of REACH ACO patients and patients who have been de-aligned from the ACO. Dealignment can occur for patients who are now deceased, have relocated, or switched to a Medicare Advantage plan. These patients will fall off the following month’s report.

Medicare makes it difficult for providers to actively remove patients from the list to ensure there is no active selection in which patients are managed in the program; this helps to prevent avoidance of underserved populations.
