Original Medicare beneficiaries that you’re already managing can be aligned by completing a Voluntary Alignment form in office, or returning the form we mailed to them directly. Original Medicare patients include those that have Medicare only, those that have a secondary or supplement in addition to Medicare. Additionally, your new Original Medicare patients will be able to align in office, once they’ve had their initial in person visit. Patients can opt in via voluntary alignment throughout the duration of the performance year and will be added to your assigned patient list on a quarterly basis. You will continue to collect fee-for-service reimbursement for any services rendered to patients until they are aligned the following quarter.

Forms from patients need to submitted 45 days prior to the start of the quarter to be aligned by that quarter. They will then be added to the platform a month after they’ve been aligned, and providers will get paid for that patient retroactively.
