Patients can begin voluntarily aligning on January 1st of the practice's first performance year via e-signature or paper form, or by choosing their primary clinician on mymedicare.gov. You must submit alignment forms 45 days prior in order to align patients for the next quarter. If you are unable to locate forms, email help@pearlhealth.com and we will send them over for you and your patients.

In the meantime, Pearl has delivered a Medicare compliant paper form to your practice to align patients via mail and digitally.
