What is the POLST alert?

Pearl's POLST alerts identify patients who may benefit from completing a Physician Orders for Life-Sustaining Treatment (POLST) form - a document that translates care preferences into an actionable medical order.

Eligibility is based on chronic condition burden, where the patient has a Charlson Comorbidity Index score of 10 or higher.

ACP Alert Status

We will be retiring the ACP alert in the Pearl platform, meaning you will no longer see the ACP alert July 1st onwards.
