Why This Change

Pearl is introducing a POLST alert that helps your practice

identify patients who may benefit from completing a

Physician Orders for Life-Sustaining Treatment (POLST)

form. The alert replaces the previous Advance Care

Planning (ACP) alert with a more focused signal.

A POLST form gives providers a concrete starting point

for the end-of-life conversation, and it produces an

actionable medical order. It is a lower-lift step than a full

advance care planning visit. Pearl still recommends ACP

conversations broadly - practices that prefer to schedule

a full ACP visit and incorporate POLST completion within

that visit may do so as well.


What's Changing on the Platform

The previous Advance Care Planning (ACP) alert is being

retired. A new “Eligible for POLST discussion” alert

replaces it in the Proactive Management section.

The change aligns the platform with best practices in

end-of-life care and planning, and it points the signal

toward a more focused, actionable step.
