Statements are accessible in PDF or excel format by navigating to the “PAYMENTS” space in the Pearl Platform, under the “Statements” tab.

Payment statements include a patient level reflection of each per beneficiary per month payment issued, beneficiary type (Aged and Disabled or End Stage Renal Disease), beneficiary date of birth, and whether or not beneficiary has opted into data sharing. There is a legend included at the bottom of the payment statement indicating reason for payment eligibility. Payment details for aligned beneficiaries will be separate from dis-aligned beneficiaries as reflected in sample statement below.

While the dollar amount awarded per beneficiary (PBPM) will remain stable for the duration of the Performance Year, the overall monthly payment will fluctuate based on number of patients assigned due to patients switching to a Medicare Advantage plan, relocating out of the service area, opting in or out of data sharing, or passing away. Adjustments usually lag by two months due to CMS processing (eg: July payment adjustments will reflect patient alignment data from May)
