How does this affect my patients?

We’re required to notify your assigned patients of your participation in this model, so they will receive a letter from us notifying them that you have voluntarily opted into the ACO REACH program, and that their benefits will not be affected. Any eligible patients that are not already assigned to your practice can opt in by signing a Voluntary Alignment form.
