Medicare assigns patients to your panel in two ways:
Claims-Based Alignment - Plurality of visits identified in your claims history dictate which patients are assigned to you at the start of the Performance Year
Voluntary Alignment - Patients complete a form that designates you as their Primary Care Provider and are added to your list of assigned patients on a quarterly basis
