Voluntary Alignment is a feature specific to your participation in Medicare's Direct Contracting model, and forms should not be extended to all patients.
Only the following patients should be invited to align by signing the VA form:
Traditional Medicare Fee-For-Service (FFS) beneficiaries
Note: VA is not applicable to Medicare Advantage, or any other payer or patient populations.
