It's in the provider's benefit to align patients early so they have a better line of sight into the health and cost driver's of that patient earlier in the year. It's important to have an Annual Wellness Visit (AWV) and get the patient appropriately coded at the outset to ensure the provider is being treated fairly in the model and avoid an artificially low risk score, thus making it more difficult to reflect accurately after the patient’s first year. Additionally, Medicare uses these diagnoses to create your “benchmark” or budget, which ultimately determines how your shared savings are calculated.
